An annotated bibliography on single subject research design
Abstract

Innovations in teaching: a learning format designed to enhance critical appraisal skills
Abstract
Aim: to create a learning experience which enhances clinical competence by encouraging critical appraisal and accurate self-assessment. Method: Strategies for enhancing participants’ appraisal skills included: -preparation of a “case study” to demonstrate clinical decision making in primary practice. -self-evaluation of that case study according to a pre-determined framework. -critiquing of case studies prepared by peers. All participants were provided with a set of guidelines for case selection, case preparation and case evaluation. Each participant prepared one case study and critiqued two others. Student anonymity was ensured by means of cases and critiques being identified through examination numbers rather than names. Results: From the student perspective the case preparation aspect of this learning experience was well accepted. The student response to peer-evaluation was more guarded while only one third of respondents considered that the self-assessment component of the exercise may potentially contribute to their development as independent learners. From the lecturer’s perspective the total exercise was worthwhile despite certain logistic difficulties. Recommendations for modifying the learning experience have been included. Conclusions: By encouraging reflective clinical decision making and engaging participants in self- and peer performance appraisal, this format creates a learning framework which provides an opportunity for students to enhance their ability to undertake critical appraisal in a context relevant to their future professional practice. But : Mettre sur pied une formation enrichissante qui met en valeur les compétences médicales tout en favorisant l’évaluation critique et l’auto-évaluation précise. Méthode : les stratégies suivantes favorisent les aptitudes critiques des participants : · l’élaboration d’une étude de cas qui permet de faire ressortir les prises de décisions médicales auxquelles seront confrontés les participants dès le début de l’exercice de la chiropratie; · l’autocritique de cette étude de cas selon le cadre établi au préalable; · la critique des études de cas menées par les pairs. Des directives générales ont été fournies à tous les participants quant au choix, à la préparation et à l’évaluation de l’étude. Chacun des participants a été appelé à préparer une étude de cas et à en critiquer deux autres. L’anonymat a pu être préservé en identifiant les études et les critiques par des numéros d’examens au lieu des noms des participants. Résultats : Les participants ont été satisfaits de la préparation de cette formation enrichissante. Les étudiants ont toutefois montré plus de réserve quant à la section consacrée à l’évaluation des pairs. Par ailleurs, seulement un tiers des sujets considèrent que la section de la formation traitant d’auto-évaluation pourrait éventuellement contribuer à acquérir de l’autonomie. Quant aux professeurs, cette expérience s’est avérée enrichissante en dépit de quelques difficultés d’ordre logistique. Des recommandations visant à modifier la formation ont été incluses. Conclusions : En encourageant une prise de décision médicale réfléchie et en impliquant les participants dans l’auto-évaluation et la critique de la performance de leurs pairs, cette formation met en valeur le cadre académique qui donne aux étudiants la possibilité d’améliorer leurs aptitudes à assumer la critique dans un milieu propre à l’exercice de leur future profession. Trauma induced osteonecrosis of the hip: a case report
Abstract
A chronic case of lower back pain in a young female is presented. A number of conditions can lead to back pain. This article discusses the sequelae of osteonecrosis of the hip and how it resulted in a leg length discrepancy leading to altered lumbo-pelvic biomechanics and its genesis of lower back pain. Diagnostic imaging and clinical manifestations are discussed in detail. Early diagnosis is important for appropriate treatment and to decrease the severity of deformity. (JCCA 1995; 39(4):210-216)

On présente un jeune femme souffrant de douleurs lombaires basses chroniques. Un bon nombre de pathologies peuvent être reliées aux douleurs lombaires. Cet article examine les séquelles reliées à l’ostéonécrose de la hanche et les causes de l’inégalité de longueur de la jambe entraînant une modification des biomécaniques lombaires et pelviens. Cet article traite aussi de l’étiologie des douleurs lombaires basses et présente une étude détaillée sur la lecture d’une image diagnostique et les symptômes cliniques. Le diagnostic précoce s’avère important afin de déterminer le traitement approprié et de diminuer la gravité de la déformation. (JACC 1995; 39(4):210-216)

Acute proximal deep vein thrombosis: presentation in a chiropractic office
Abstract
The diagnosis of acute proximal deep vein thrombosis (DVT) can be difficult to make on the basis of clinical findings alone. Since it may be encountered by all clinicians, chiropractors must be aware of the often confusing clinical picture with which it may present. In its early stages patients may present with posterior thigh, calf and groin pain, symptoms that may mimic other types of biomechanical disorders. Because 50% of patients with DVT may develop life threatening thromboembolism, early detection and appropriate referral is crucial. To highlight the clinical presentation of DVT and the phenomenon of “silent embolism” the case of a 58-year-old male presenting to a chiropractic office for assessment and treatment of low back and leg pain is presented. The prevalence, etiology and risk factors associated with its onset are reviewed. Physical examination findings, diagnostic imaging and treatment are discussed. Considerations in the chiropractic management of the post-thrombolytic patient are highlighted. (JCCA 1995; 39(4):205-209)

Il peut s’avérer difficile d’établir le diagnostic de la thrombose veineuse profonde aiguë proximale en se basant uniquement sur les résultats cliniques. Etant donné que tous les cliniciens peuvent être confrontés à ce diagnostic, les chiropraticiens doivent être conscients qu’ils peuvent faire face à un tableau clinique souvent confus. Au stade précoce, le patient peut souffrir d’une douleur de la cuisse postérieure, au mollet et à la hanche et présenter des symptômes similaires à d’autres types de troubles biomécaniques. Le repérage précoce de la thrombose veineuse profonde et l’orientation du sujet vers un spécialiste s’avèrent essentiels étant donné que dans 50 pour cent des cas, cette maladie évolue en thromboembolie. Le cas d’un homme de 58 ans se présentant à un chiropraticien afin d’évaluer et de traiter ses douleurs du bas du dos et des jambes souligne le tableau clinique de la thrombose veineuse profonde et du phénomène de l’embolie asymptomatique. On analyse la prévalence, l’étiologie et les facteurs de risque reliés à l’apparition des premiers symptômes. On examine le résultat de l’examen physique et la lecture de l’image diagnostique. L’observation des soins chiropratiques prodigués au patient post-thrombolytique est soulevée. (JACC 1995; 39(4):205-209)
