The dental amalgam controversy: a review
Abstract
In spite of the long history of mercury amalgam as a dental restorative material, its use continues to be controversial. Mercury vapour is continuously released from dental amalgam and is ultimately absorbed into a variety of tissues. Experimental data have demonstrated that the uptake, tissue retention and excretion of mercury from dental amalgam is significant. Evidence has accumulated indicating a relationship between tissue mercury levels and a multitude of clinical manifestations. However, the clinical significance of mercury toxicity from dental amalgams is a matter for debate. The literature is devoid of randomized clinical trials that are rigorously designed to address this issue. Thus, although research data renders the notion of amalgam safety questionable, the dental community appears determined to continue its use as long as unequivocal evidence correlating amalgam mercury toxicity to specific clinical conditions is lacking. En dépit de l’historique étendu de l’amalgame au mercure à titre de matériel de restauration dentaire, son utilisation demeure controversée. L’amalgame dentaire rejette continuellement des vapeurs de mercure qui, en bout de ligne, sont absorbés par une variété de tissus. Les données expérimentales ont démontré que l’absorption, la rétention par les tissus et l’excrétion du mercure à partir des amalgames dentaires étaient significatives. Des preuves ont été accumulées indiquant un relation entre le niveau de mercure des tissus et une multitude de manifestations cliniques. Cependant, la signification clinique de la toxicité du mercure à partir des amalgames dentaires reste un sujet de débat. La documentation à ce sujet est dépourvue d’études cliniques réparties au hasard qui sont rigoureusement conçues pour évaluer ce problème. Donc, bien que les données des chercheurs remettent en question la sécurité des amalgames, la communauté dentaire semble déterminée à continuer d’utiliser ces derniers tant qu’aucune preuve catégorique mettant en corrélation la toxicité de l’amalgame au mercure et certaines conditions cliniques ne sera fournie. A mail survey of health care professionals: an analysis of the response
Abstract
Mail questionnaires provide a cost effective alternative for the survey of geographically dispersed populations. However, mail questionnaires are also characterized by low response rates, particularly for medical and other professionals. As a result of the potential systematic differences between respondents and nonrespondents, the external validity of the results are jeopardized. A number of techniques, such as follow-ups, have been developed in order to improve the response rates while at the same time retaining the cost advantage of the mail questionnaire. The present article is a discussion of a national survey of chiropractors. Two follow-ups, a postcard reminder, and a second questionnaire were utilized in the survey. An overall response rate of slightly less than sixty-nine (68.78) percent was achieved. These results indicate that satisfactory and realistic response rates can be achieved with health care professionals who are often viewed as being resistant to surveys. (JCCA 1996; 40(3):162-168)

Les questionnaires posés constituent une alternative rentable pour enquêter sur les populations géographiquement dispersées. Cependant, le questionnaire médical se caractérise aussi par un faible taux de réponse, en particulier pour les professionnels du secteur médical et les autres professionnels. Etant donnés les différences potentielles systématiques entre les répondants et les non-répondants, la validité externe des résultats est compromise. Certaines techniques, telles que le suivi, ont été mises au point pour améliorer le taux de réponse tout en maintenant la rentabilité du questionnaire postal. Cet article est une discussion portant sur une étude nationale effectuée auprès des chiropraticiens. Le suivi, une carte rappel et un second questionnaire ont été utilisés dans cette enquête. Un taux de réponse global légèrement inférieur à 69 pour cent (68,78) a été obtenu. Ces résultats démontrent que les taux de réponse satisfaisants et réalistes peuvent être obtenus auprès des professionnels de la santé souvent considérés comme des personnes s’opposant aux études. The chiropractic college admission test: a proposal for its development and use
Abstract
A cardinal characteristic of any profession is self-regulation. It is argued in the present paper that chiropractic has now reached a level of professional maturity that indicates the need for the final aspect of self-regulation: a standardized selection approach into professional schools or colleges. Quality control of membership can then begin at the entry point into the profession. An admission test – the Chiropractic College Admission Test (CCAT) – is proposed and outlined for use for the selection of candidates into chiropractic colleges. Such a test would be beneficial for students applying to the colleges, regulatory and licensing boards, to the profession as a whole, to the chiropractic colleges, to other professions, and to government as well as the general public. The proposed CCAT contains elements that are general to many health professions such as knowledge of the biological and physical sciences, verbal and linguistic reasoning and visual perceptual ability. The test, however, is proposed to have elements that are unique to chiropractic. Based on the performance of other admission tests (e.g. Dental Admission Test, Medical College Admission Test), it is argued that the CCAT could be constructed and used to have the highest technical properties of validity and reliability. Such a test would become an integral tool in maintaining quality assurance, beginning at the earliest point of the profession. (JCCA 1996; 40(3):150-161)

L’auto-réglementation est une caractéristique importante de toute profession. On explique dans cet article que la chiropratique a maintenant atteint un tel niveau de maturité professionnelle qu’il est nécessaire de passer au stade ultime de l’auto-réglementation en créant un moyen de sélection standard à l’entrée des écoles et des collèges. Le contrôle de la qualité des membres pourrait donc commencer au point d’entrée de la profession. On suggère un test d’admission – le test d’admission du chiropratique college (CCAT) – qui serait utilisé pour sélectionner les candidats à l’entrée des collèges de chiropratique. Un tel test serait bénéfique pour les candidats qui présentent leur candidature dans les collège, pour les offices de réglementation et d’évaluation, pour la profession toute entière, pour les collèges de chiropratique, pour les autres professions, pour le gouvernement et pour le grand public. Le CCAT proposé contient des éléments communs à plusieurs professions de santé (connaissance des sciences biologique et physique, raisonnement verbal et linguistique et capacité de perception visuelle). Toutefois le test propose d’intégrer des éléments uniques à la chiropratique. En se basant sur les performances d’autres tests d’admission (p. ex., le Test d’admission dentaire, le test d’admission au collège médical), le CCAT pourrait être conçu et utilisé de façon à intégrer les plus hautes propriétés techniques de validité et de fiabilité. Un tel test pourrait devenir un outil intégral permettant de maintenir l’assurance de la qualité, en commençant au point d’entrée de la profession. (JACC 1996; 40(3):150-161)

Spinal manipulation in a case of sacral fracture: presentation in a chiropractic office
Abstract
A case is presented involving the successful management of symptoms following a sacral fracture. Treatment was primarily comprised of sacroiliac joint manipulation. The importance of accurate diagnosis, realistic risk/benefit assessment, and appropriate treatment is emphasized in considering contraindications of spinal manipulative therapy. Il s’agit d’un cas relatant le traitement réussi des symptôms reliés à une fracture sacrale. Le traitement reposait essentiellement sur des manipulations de l’articulation sacro-iliaque. L’importance d’un diagnostic précis, d’une évaluation réaliste des risques/bénéfices et d’un traitement adapté est mise en relief, compte tenu des contre-indications d’un traitement basé sur des manipulations vertébrales.
