Legislative approaches to the regulation of the chiropractic profession
Abstract
Traditional or complementary health care services have a growing and significant role in both developed and developing countries. In the United Kingdom there was new legislation to regulate the chiropractic and osteopathy professions in 1993 and 1994. This was supported by the British Medical Association. There is now legislation regulating chiropractic practice in many jurisdictions. In general chiropractic is regulated as a separate discipline, although chiropractic and osteopathy are jointly regulated in some Australian states and South Africa. Regulatory bodies generally have chiropractic and lay representation, and sometimes medical representation. In Canada and the United States there is an emphasis on a defined scope of practice. In the UK, Australia, New Zealand and Hong Kong there is no definition of scope of practice and an emphasis on protection of title. This paper reviews the different approaches to regulation of the chiropractic profession. Subjects discussed include the composition of regulatory boards, scope of practice including access to diagnostic services, educational requirements and protection of title. Les services de santé traditionnels ou complémentaires jouent un rôle croissant et considérable à la fois dans les pays développés et dans les pays en voie de développement. Au Royaume-Uni, on a introduit de nouvelles lois en 1993 et 1994 pour réglementer les professions de chiropraticiens et d’ostéopathes. Cette réglementation a été appuyée par la « British Medical Association ». Il existe maintenant une législation qui réglemente la pratique de la chiropractie dans plusieurs juridictions. En général, la chiropractie est réglementée comme une discipline à part, même si la chiropractie et l’ostéopathie sont réglementée conjointement dans certains états australiens et en Afrique du Sud. Les organismes chargés de la réglementation sont en général représentés par des chiropraticiens et par des non-professionnels, et parfois des médecins. Au Canada et aux Etats-Unis, on met l’accent sur un cadre d’exercice bien défini. Au Royaume-Uni, en Australie, en Nouvelle-Zélande et à Hong Kong, il n’y a aucune définition du cadre d’exercice et on insiste sur la protection du titre. Cet article passe en revue les différentes approches choisies pour réglementer la profession de chiropraticien. Divers sujets sont abordés, tels que la composition des comités chargés de la réglementation, le cadre d’exercice y compris l’accès aux services de diagnostic, et les exigences pédagogiques et la protection du titre. The effect of trochanteric support on low back strength: a pilot study
Abstract
Study Design. An experiment was conducted regarding the effects of wearing a trochanteric support on patients with low back pain. Objective. To evaluate the effectiveness of a trochanteric support on the pain and strength parameters of patients with low back pain. Summary of Background Data. No study has examined the relationship between a trochanteric support and its’ effect on pain and strength of the lumbar spine. Method. Five patients with diagnosed low back strain/sprain injuries, were monitored via the VAS and the Arcon-ST static strength testing system. Heart rate was monitored to determine performance at maximal level. Patients were tested pre post trochanteric support – Spine Power Belt. Results. Post testing showed the following: Improved strength averaged 35% for the near lift and this was significant at the p< 0.001 level. For the high lift, strength improved by 27% and this was significant at the p<0.05 level. VAS scores were improved by 15%. Conclusion. This pilot study suggests early intervention with a trochanteric support increases the strength of the back significantly and may allow patients to advance more quickly through their low back rehabilitation program and return to work. Méthodologie. On a mené une expérience sur les effets du port d’une ceinture trochantérienne chez les patients souffrant d’une lombalgie. Objectif. Evaluer l’efficacité d’une ceinture trochantérienne sur les paramètres de douleur et de force des patients souffrant de lombalgie. Résumé des données de base. Aucune étude n’a examiné la relation entre une ceinture trochantérienne et ses effets sur la douleur et la force de la colonne lombaire. Méthode. Cinq patients présentant un diagnostic de foulure/entorse lombaire furent étudiés au moyen de l’échelle analogique visuelle et du système de test de la force statique Arcon-ST. La fréquence cardiaque fut également suivie afin de déterminer la performance au niveau maximal. Les patients furent testés avant et après le port de la ceinture trochantérienne (Spine Power Belt). Résultats. Les tests réalisés après le port de la ceinture ont donnés les résultats suivants: l’amélioration de la force atteignait en moyenne 35% pour le relèvement de près (« near lift ») et ceci était significatif au niveau p<0,001. Quant au relèvement élevé (« high lift »), la force s’est améliorée de 27% et ceci était significatif au niveau p<0,05. Les scores de l’échelle analogique visuelle furent améliorés de 15%. Conclusion. Cette étude pilote suggère que l’intervention précoce avec une ceinture trochantérienne augmente grandement la force du dos et peut permettre aux patients de progresser plus rapidement dans leur programme de rééducation de la colonne lombaire, et de retourner au travail. Rehabilitation – an addition to standard chiropractic management for chronic low back pain
Abstract
A case of chronic vertebrogenic low back pain of biomechanical origin characterized by intermittent radiculitis and truck muscle insufficiency is presented. Initial allopathic and chiropractic management provided only palliative relief. A three-month program of in-office rehabilitation including progressive/resistance exercise was administered in conjunction with spinal manipulation. This program proved effective in reducing the patient’s low back pain and dependency on passive care. (JCCA 1996; 40(2):100-103)

Cet article présente un cas de lombalgie vertébrogénique chronique d’origine biomécanique caractérisé par une radiculite intermittente et une insuffisance musculaire du tronc. Les traitements allopathiques et chiropratiques initiaux n’ont procuré qu’un soulagement palliatif. Un programme de trois mois de rééducation en cabinet comprenant des exercices progressifs contre résistance fut administré en conjonction avec des manipulations vertébrales. Ce programme s’est révélé efficace dans la réduction de la lombalgie du patient et de sa dépendance aux soins passifs. (JACC 1996; 40(2):100-103)

Historical Review CMCC’s persistent pursuit of university affiliation Part III: the push for union with the University of Victoria, BC, 1988 to 1992
Abstract
The period between 1988 and 1992 is reviewed with respect to the Canadian Memorial Chiropractic College (CMCC) and its attempted affiliation with the University of Victoria in British Columbia. Part I, published earlier, detailed the period between 1945 and 1969 and the proposals for university affiliation with the University of Alberta and Brandon College in Manitoba. Part II focused on the period between 1969 and 1988 and discussed government inquiries, strategic planning and political intervention. In Part III of this triad, the chronology of events with respect to CMCC’s sustained, sophisticated and focused attempt at affiliation with the University of Victoria is discussed and the problems encountered with the Senate are described which lead to the eventual breakdown of any potential union. La période comprise entre 1988 et 1992 est examinée relativement au Canadian Memorial Chiropractic College (CMCC) et à sa tentative d’affiliation à la Victoria University de la Colombie-Britannique. La première partie publiée précédemment, étudiait la période comprise entre 1945 et 1969 et les propositions d’affiliation à la Alberta University et au Brandon College du Manitoba. La deuxième partie porte sur la période comprise entre 1969 et 1988 et relate les enquêtes du gouvernement, la planification stratégique et l’intervention politique. La troisième partie de cette trilogie suit de façon chronologique les événements relatifs à la tentative d’affiliation du CMCC avec l’Université de Victoria, tentative que l’on peut caractériser de persévérante, sophistiquée et ciblée. Cette section aborde également les problèmes rencontrés avec le Sénat, qui mèneront finalement à la rupture de toute union éventuelle. (JACC 1996; 40(2):87-99)

Hip and buttock pain due to occult pelvic fractures: a case report
Abstract
Pelvic fractures are a relatively uncommon presentation to a chiropractic office. A case is presented of pelvic fractures following a motor vehicle accident that initially went undetected despite a set of lumbar spine and pelvic radiographs. The condition was only discovered when the patient continued to suffer hip and buttock pain that was not improving in a reasonable period of time. Follow-up radiographs revealed well healing, stable pelvic fractures. The classification of pelvic fractures as well as the importance of a high index of suspicion in dealing with patients who have been involved in motor vehicle accidents is discussed. The importance of obtaining an accurate diagnosis and its relevance to duration of disability is highlighted. (JCCA 1996; 40(2):82-86)

Les cas de fractures pelviennes se présentent relativement peu fréquemment dans un cabinet de chiropractie. Cet article présente un cas de fractures pelviennes survenues après un accident automobile et qui n’ont pas été détectées à l’origine malgré la série de radiographies de la colonne lombaire et du pelvis. Le problème a été découvert lorsqu’on s’est aperçu que le patient continuait de souffrir de pygalgie et de coxalgie et que les douleurs ne s’étaient pas améliorées sur un laps de temps raisonnable. Des radiographies de suivi ont montré des fractures pelviennes stables en bonne voie de guérison. La classification des fractures pelviennes ainsi que l’importance du degré de suspicion en présence de patients impliqués dans des accidents automobiles sont décrites. L’importance d’un diagnostic précis et ses conséquences sur la durée d’invalidité sont mises en relief. (JACC 1996; 40(2):82-86)

Nutrient canals of the ilium: a case report
Abstract
The nutrient canals of bone are a frequent finding on skeletal radiographs. These canals may by misinterpreted as fracture lines especially in patients who present to a chiropractic office with a history of trauma. The case of a 22-year-old female with a nutrient canal in the ilium is briefly reviewed. (JCCA 1996; 40(2):79-81)

On observe fréquemment les canaux nourriciers de l’os lors d’une radiographie osseuse. Ces canaux peuvent être confondus avec des traits de fracture, particulièrement chez les patients qui se présentent au cabinet avec un antécédent de traumatisme. Cet article relate le cas d’une femme de 22 ans présentant un canal nourricier dans l’os iliaque. (JACC 1996; 40(2):79-81)
